Health check-up for traveling abroad.

A health check-up for traveling abroad either for business or education is important as it is a requirement of traveler screening in each country. The health check-up for traveling abroad should be done by a standard hospital.

The King Chulalongkorn Memorial Hospital, the Thai Red Cross Society, is considered one of the 4 hospitals in Bangkok certified for health examinations and can issue health examination certificates for every country around the world regardless of which country you will travel to. Accordingly, the specific health check-up is set up for a convenient and rapid service.

Health check-up recommendations for foreign countries.

  1. Contact the automatic service kiosk, in the front of Pro Por Ror Building. Select ‘Health check-up for traveling abroad’.
  2. The newcomer should fill in the green form. Existing customers, please fill in the yellow form, ticking ‘Health check-up for traveling abroad’ and attach a copy of your passport (For information document in English).
  3. Contact Card counter numbers 8-13 for registration.
  4. Contact the automatic service kiosk, medicine and social prevention on floor 16, Por Por Ror Building.
  5. Contact room 18, zone D with the request form, (green or yellow), doctors form, 2 copies of your passport document and hospital ID card for further health check-up processing. 

Required Documents for a Health Check-up for traveling abroad.

  1. 3 copies of your passport document (if not, ID can be used as a replacement).
  2. 1-inch or 2-inch looking straight ahead portrait photographs (taken within the last 6 months) or 1-2 sizes depending on the form taken.
  3. Medical certificate form (In case the hospital form is not available).
  4. Examination fees according to the form for each desired country.
  5. The medical certificate is issued within 3 days (per regular examination results).
